By submitting this form electronically, you certify the provided information is truthful and accurate and that you have purchased and installed the identified equipment or completed the identified project. Utility reserves the right to withhold a rebate if any entries are found untruthful or inaccurate and may take extraordinary steps to validate the claims.

Proof of purchase is required and you will be prompted to upload a proof document into this application. The price of the efficient product/project for which you are applying for a rebate must be clearly shown separate from other charges. Utility reserves the right to reduce or deny the rebate if the proof document shows the equipment was purchased or the project was fulfilled at a substantial discount to standard rates. For the recycling rebate, proof of delivery to a landfill or recycling company is required.  Failure to provide the correct documentation will result in an application being sent back or denied.

After this application is received, Utility may elect to inspect the equipment or project to verify it qualifies. 

You must be a customer of Utility, be eligible to participate in the rebate program, and energy efficiency funds must be available at the time you submit your application. New equipment must be purchased outright, and the claim must be filed within 90 days of the installation date and not after January 31st of the following year. Utility reserves the right to change, modify, or end any or all parts of the rebate program without notice. 

Utility has no control over exogenous energy use factors including, but not limited to, consumer behavior and weather conditions, and cannot guarantee the high-efficiency equipment will result in reduced usage or in cost savings. Utility does not endorse any manufacturer, contractor, dealer, or design. Utility makes no warranties or representations of any kind regarding the equipment or any services provided in connection with this rebate program.

Utility liability will not exceed the amount of the rebate, and Utility is not liable for any equipment or services provided in connection with this rebate program, and in no event is Utility liable for any special, incidental, or consequential damages in connection with this rebate program. You agree to indemnify, defend, hold harmless and release Utility from any claims, damages, liabilities, costs and expenses (including reasonable attorneys’ fees) arising from or relating to the removal, disposal, installation or operation of any equipment or related materials in connection with the programs described in this application, including any incidental, special or consequential damages.
