By submitting this form electronically, you certify the provided information is truthful and accurate and that you have purchased and installed the identified equipment. Utility reserves the right to withhold a rebate if any entries are found untruthful or inaccurate and may take extraordinary steps to validate the claims.

Certain supporting documentation will be required, and you will be prompted to upload those documents into the file. All newly purchased items will require a dated proof of purchase. All recycled items will require a dated proof of delivery to a landfill or recycling company. Some items will also require proof of the energy efficiency claim such as an ENERGY STAR label, AHRI certificate, and/or HVAC system operating performance report (i.e. HVAC SAVE recognition). Failure to provide documentation will result in an application being sent back or denied.

You must be a customer of Utility, be eligible to participate in the rebate program, and energy efficiency funds must be available. The rebate amount is limited to 50% of the customer’s incremental installed cost for the high efficient option. New equipment must be purchased outright, and the claim must be filed within the applicable program dates.  If the installation was made in a new home and the homebuilder received a new construction rebate, an application for an equipment rebate will be denied.

Utility has no control over exogenous energy use factors including, but not limited to, consumer behavior and weather conditions, and cannot guarantee the high-efficiency equipment will result in reduced usage or in cost savings. Utility does not endorse any manufacturer, contractor, dealer, or design. Utility makes no warranties or representations of any kind regarding the equipment or any services provided in connection with this rebate program.

Utility liability will not exceed the amount of the rebate, and Utility is not liable for any equipment or services provided in connection with this rebate program, and in no event is Utility liable for any special, incidental, or consequential damages in connection with this rebate program. You agree to indemnify, defend, hold harmless and release Utility from any claims, damages, liabilities, costs and expenses (including reasonable attorneys’ fees) arising from or relating to the removal, disposal, installation or operation of any equipment or related materials in connection with the programs described in this application, including any incidental, special or consequential damages.
